Welcome back. To help NHS Test and Trace teams, we need a couple of details about your visit so they can get in touch, should you come into contact with a new COVID-19 case.
We're required to keep hold of your data for 21 days after your visit. It'll only be used where necessary to help stop the spread of COVID-19 and after those 21 days, we'll delete it.
After you have completed the form, please keep your confirmation page open as staff may ask you to show this when you enter the Centre.
